How to Collect Your Family Medical History in One Evening
What to write down for each relative, whom to ask, and why the age an illness began matters more than the diagnosis. You can gather a family medical history in an evening.

“What illnesses run in your family?” Most people stumble on this question at the doctor’s office. We know our relatives’ birthdays by heart, but not what our grandfather was ill with or at what age. Yet a family medical history can sometimes help catch a risk years earlier. You can collect one in a single evening.
What to write down for each blood relative
Three things. Serious illnesses (first of all heart and blood vessels, blood pressure, diabetes, cancer). The age at which they began. And the cause of death, if the person is no longer alive. Age matters more than the diagnosis itself: a heart attack at 45 and at 85 mean completely different things for assessing risk.
Whom to ask
Start with the close circle: parents, full brothers and sisters, grandparents. Then great-grandparents, if you can. Ask specifically. Not “what was he ill with”, but “what did he die of”, “at what age did the blood pressure appear”, “did anyone have cancer, and who, and when”.
No one may remember the exact diagnoses, and that is normal. Write it as it is said: “heart trouble”, “sugar”, “a tumor”. Even a general wording with an age works as a landmark for a doctor.
Keep the maternal and paternal lines separate
Record the mother’s and father’s sides separately. That way you can see at once whether something repeats in several relatives. Repetition, not a single case, is the main signal.
What this is, and what it is not
This is not a diagnosis and not a reason to worry. It is a map you build once and bring to your doctor, instead of panic and phone calls to relatives just before the appointment. If something in it worries you, that is a reason to ask your doctor.
In Heritage AI you can keep this history by relative and export it for your doctor as a single file. To get started, see the free research plan.
Questions and answers
- What should a family medical history include?
- For each blood relative: serious illnesses (heart and blood vessels, blood pressure, diabetes, cancer), the age they began, and the cause of death if the person has died.
- What if my relatives do not remember the exact diagnoses?
- Write down what they can say, such as “heart trouble”, “sugar” or “a tumor”, together with an age if possible. Even a general wording is a useful starting point for a doctor.
- Why is the age of onset more important than the diagnosis?
- Because the same illness means different things at different ages. A heart attack at 45 and at 85 say very different things about family risk. Your doctor can interpret this; the notes are only a map to bring.
- Is a family medical history a diagnosis?
- No. It is a set of notes about your relatives’ health. If something in it worries you, talk to your doctor.
